He was injured during the match, then died afterwards as a result of the in-ring injury. Very tragic.
Here's video leading up to what you posted.
I am guessing that 1 of 3 things happened. Either: 1) the kick to the head was accidentally legit, and it took a few seconds for the injury to take its toll, until he collapses on the ropes (I don't think this was it, there was still spring in his step after this bump) 2) when he flies out of the ring, perhaps he takes a hard bump we don;t see, the has a few seconds of consciousness before collapsing onto the ropes, or 3) he lands into the ropes too hard and does...something...to cause the injury leading to death?
The whole thing seems weird, and I haven't read anymore about it since this morning, so I don't know if more details were released about how it happened, but as of this morning, that was the gist I gathered from what I saw. Edit: Read the cause of death was "cervical spine trauma", so it must have been a neck-jerking into the ropes type thing. Uncool. And as much as the crowd HATED that Rey missed the 619, it's a damned good thing he did. If he landed that there would probably be no doubt in his mind that he was responsible for killing the guy.

I highlighted the most important part of the report, which was the incident itself. It's one of those moments where it's just a fluke accident. First off, I think Aguayo took the wrong way into landing on the second rope, which was him already having an arm on the rope, where both arms are supposed to protect you when you land on it so you can safely rest your neck on the rope. He landed neck-first on the rope, and those ropes aren't soft on the inside, especially ropes like those. When Manik hit the rope was even worse, because he hit that rope harder than you should have, which would probably hurt anyone on it, so it aggravated the neck even more, which caused this whole incident. It's just good that Mysterio and Manik tried to end the match as good as they possibly could.
This is the whole risk in Lucha Libre. More high flying and more risky moves will lead to many injuries, and in this unfortunate case, a death. That's the main reason why Lucha Libre hasn't really transcended past Mexico, and professional wrestling has been a mainstay in the United States and Europe. It's very unfortunate that it happened to him, no doubt about that. It's one of the risks of doing Lucha Libre, though.

After watching it (came here to share and noticed it already was posted) I think that Mysterio missed on purpose. Seemed like the other wrestler may have said something to tell him the other guy isn't okay, hence the weirdness of the 619 that seemed blatantly avoided. Very messed up, regardless.

What you misunderstood from Twitter was that Rey fought WITH Aguayo DURING his first match. This was definitely not Aguayo's first match. They just meant that he and Rey had worked together before. Many times.
